How do I get started with securing home care services?

This could be via a doctor’s referral or direct client/family consultation, assessment, and care planning., followed by direct contact.

What types of medical professionals are providing home care at Harmony Care Haven?

We have professional caregivers, CAN, LPN, and RNs that provides home care tailored to the client’s needs.

How often will a caregiver visit?

Caregivers’ visits is as required and agreed based on the care plan and the patient’s needs.

Can home care include help with daily activities like bathing and dressing?

Personal care is definitely part of the services offered.

How is the care plan developed and adjusted?

The care plan is developed in line with either the doctor’s referral, or in conjunction with the client or their family, based on the client’s condition.

How much does home health care at Harmony Care Haven cost?

Service costs are agreed based on the mode of payment.

Does insurance cover Harmony Care Haven home health care services?

Both private pay and long-term care insurance are accepted.

Are there any additional costs I should be aware of?

Once the insurance is fully accepted, there are no out-of-pocket expenses to incur.

How are caregivers screened and trained?

All Sycamore Oasis caregivers are screened before they are hired, including background checks, qualification validation, and ongoing training

What measures are in place to ensure the safety and well-being of patients?

Caregivers are trained in safety protocols, emergency procedures, and infection control for our clients. 

Can I contact someone if I have concerns about the care provided?

Our clients are provided with detailed information (phone number), on how to reach the care coordinator or customer service for assistance

How does the service monitor and ensure the quality of care?

Our Admin Manager regularly visits our clients to conduct assessments and asks for feedback on the quality of care being provided. 

What happens if a caregiver cannot make a scheduled visit?

We anticipate disruptions due to caregiver related emergencies and put in place adequate backup plans to ensure continuous care.

What legal documents are needed for home health care?

Medical records from doctor’s referral, power of attorney, and advance directives.

How is patient privacy protected?

We provide services fully consistent with HIPAA compliance caregiver training that emphasizes protection of patient information.

What is the process for discontinuing services if needed?

Our services can be ended at any time with 24 hours’ notice.

Can services be adjusted if my loved one’s condition changes?

Yes, our plans can be modified as the client’s needs or diagnosis requires or evolves.
